Program Indicator Reports - FY 2008

The following are links to SnapSurvey forms to be filled out by Extension educators. If you have any problems accessing or filling out these survey forms, please contact Gitta Jenkins. These surveys should be completed within 15 days of the end of the quarter (see due dates listed below) .
Please only fill out those surveys that have data reflecting programs in which you, your county, or program unit participated.
The Excel spreadsheet provided below is to help you track your work during the quarter. However, the spreadsheet is only a worksheet—a tool, not a report form. At the end of each quarter, please enter each program’s data into the Snap Survey report. Do not send Excel worksheets to Paul Becker or Gitta Jenkins.

Data will be posted by the end of the month following each quarter. Click on the word "Data" next to the program name to see the Excel file for that program.
Due dates:
Quarter 1 (October 1 thru December 31, 2007): January 15, 2008
Quarter 2 (January 1 thru March 31, 2008): April 15, 2008
Quarter 3 (April 1 thru June 30, 2008): July 15, 2008
Quarter 4 (July 1 thru September 30, 2008): October 15, 2008
